Healthy Community

Improving Health and Well-Being for All

A healthy community starts with stability. When families have a safe place to call home and access to the support they need, everything else becomes possible. Our work focuses on addressing the root causes of hardship so people can stay housed, stay healthy and build a stronger future.

Explore how housing stability and proactive support are building healthier, more secure communities.

Housing Navigation

Housing is foundational to a person’s health. Through California Advancing and Innovating Medi-Cal (CalAIM), our Housing Navigation services connect Medi-Cal members who are experiencing or at risk of homelessness with the tools they need to find and maintain stable housing.

We support individuals and families by providing:
• Help searching for safe and appropriate housing
• Coaching to build long-term housing sustainability
• Connections to local partners and community resources
• Ongoing follow-up to ensure people remain stably housed

Every step is focused on helping people regain safety, stability and a sense of control in their lives.

Solano Housing Programs

Housing challenges can happen at any stage, whether you’re struggling to stay housed or searching for a place to call home. Our Solano Housing Programs connect residents with personalized support to help prevent homelessness, access housing opportunities, and build long-term stability.

Programs and services may include:

• Housing stabilization support and resource navigation
• Housing search and application assistance
• Landlord coordination and housing advocacy
• Case management and supportive services
• Connections to community resources that promote long-term stability

By meeting people where they are, we help individuals and families overcome housing barriers, secure safe housing, and create a pathway toward lasting stability.

Adult Reentry Program

United Way’s Adult Reentry Program provides care management and housing navigation services to support individuals returning to the community after incarceration. The program helps participants successfully reintegrate and build long‑term stability.

Services are personalized to each participant’s goals and needs, including:

  • Health and behavioral health services
  • Education and job training
  • Substance use support
  • Vocational skills development
  • Financial wellness
  • Transportation assistance
  • CalAIM programs
  • Housing navigation + rental assistance support

Our team works closely with each participant to remove barriers, reconnect them to needed services, and help them stabilize long‑term.
