Housing Navigation

Housing is foundational to a person’s health. Through California Advancing and Innovating Medi-Cal (CalAIM), our Housing Navigation services connect Medi-Cal members who are experiencing or at risk of homelessness with the tools they need to find and maintain stable housing.

We support individuals and families by providing:
• Help searching for safe and appropriate housing
• Coaching to build long-term housing sustainability
• Connections to local partners and community resources
• Ongoing follow-up to ensure people remain stably housed

Every step is focused on helping people regain safety, stability and a sense of control in their lives.


Keep People Housed – Solano

When families fall behind on rent or utilities, even a small setback can turn into a crisis. The Keep People Housed – Solano program steps in before that crisis becomes homelessness.

This program helps Solano County residents stay in their homes through:
• Emergency rent and utility support
• Legal referrals to protect tenant rights
• Personalized case management and financial coaching
• Navigation to local resources that promote stability

By addressing immediate needs and strengthening long-term planning, the program gives families the breathing room they need to heal, rebuild and move forward.


Adult Reentry Program

United Way’s Adult Reentry Program provides care management and housing navigation services to support individuals returning to the community after incarceration. The program helps participants successfully reintegrate and build long‑term stability.

Services are personalized to each participant’s goals and needs, including:

  • Health and behavioral health services
  • Education and job training
  • Substance use support
  • Vocational skills development
  • Financial wellness
  • Transportation assistance
  • CalAIM programs
  • Housing navigation + rental assistance support

Our team works closely with each participant to remove barriers, reconnect them to needed services, and help them stabilize long‑term.
