If your agency has a completed and active their Big Day of Giving profile, you only need to complete the rest of this form. If you do not apply for or have an active their Big Day of Giving profile, United Way will require  your most recent Audited Financial Report or financial statements covering the fiscal year ending June 30th 2015 or latter, and IRS Form 990 for the same fiscal year as the Audited Financial.  If these items are not readily available  on-line (your website  or www.guidestar.org ), United Way staff will contact you to get these documents.

IN-KIND

From time to time, United Way is approached by companies, community members, donors, and others with the offers of in-kind donations of new and use items . In the past, these have included school and office supplies, furniture, household items, clothes, books, and other items. To help get these donations to the organizations that needs them.
